Insurance Information

We participate with Medicare, Blue Cross/Blue Shield, and most other HMO, PPO and indemnity insurance plans. Patients who have a medical insurance plan that requires a referral from their primary care physician are required to bring the necessary documentation to our office at the time of their appointment. Co-payments for Medicare, HMO and PPO plans are due at the time of service. Patients who have insurance in which we do not participate, or who do not have any health insurance, will be required to pay at the time of service unless prior arrangements have been made with the billing department.
